The community remains the largest setting for ventilator-assisted individuals in Ontario, with over 6000 non-invasively and close to 200 invasively vented individuals living both at home and in
the province's
attendant care programs.
The PLTVS seeks to reduce pressure on ICUs and inpatient LTV programs by creating capacity for ventilator assisted individuals to be able to transition safely to the community,
and by improving the resources and education available to support those already living in the community.
The
LTV to Community discharge checklist is a useful tool to assist clinical
teams in transitioning ventilator assisted individuals from an inpatient LTV setting to the community.
The Ventilator Equipment Pool (VEP) is a provincial service operated by Kingston Health Sciences Centre
that provides ventilator equipment to individuals in the community as well as education, and is a key partner of the PLTVS.
